‘Significant failures’ led two NSW foster children to be placed with serial killer, review finds

A review found significant failures in the New South Wales Department of Communities and Justice led to two foster children aged 12 and 14 being placed with convicted triple murderer Regina Arthurell. Two staff members were suspended following the damning review, which revealed the department was warned in December about Arthurell's presence in the home.
