A coroner found Dr David Jackson's 'grossly irresponsible prescribing' directly contributed to the deaths of two drug-dependent patients in Tasmania between 2016 and 2017. The inquest revealed he provided an 'effectively unlimited supply for a drug binge' to one addict.
