The Canadian Transportation Safety Board's investigation into the Titan submersible's failure identified severe engineering and design flaws that directly contributed to its catastrophic implosion. The report highlights risky decisions and oversights at multiple stages of the project.

In 2023, the Titan submersible imploded during an expedition to the Titanic wreckage.

OceanGate's Titan submersible operated without federal regulatory oversight despite interactions with Canadian federal departments and agencies. Canada's Transportation Safety Board identified this issue and issued six recommendations to address regulatory gaps.

The Transportation Safety Board will release a final report on the Titan submersible's implosion, which occurred off Newfoundland and Labrador, resulting in five fatalities. The report is set for release almost three years after the incident.

Christine Dawood, widow of Shahzada Dawood and mother of Suleman Dawood, described receiving their remains in small 'shoebox' containers nine months after the 2023 Titan submersible implosion. The disaster killed five people, including OceanGate CEO Stockton Rush and Titanic expert Paul-Henri Nargeolet, as the submersible imploded during a dive toward the Titanic wreck.
